WHAT WE DO

myHealth for Teens & Young Adults clinic-based programs offer youth and young adults, ages 12-26, comprehensive, preventative and youth-focused medical services, mental health services, community outreach and education, and nurse home visiting for pregnant and parenting teens. Our clinic is located downtown Hopkins and our services are offered without judgment in a caring and safe environment.

JOB SUMMARY

The Clinic Health Mentor position is responsible for providing a range of sexuality education programming and serving as a resource and support around sexuality and health topics within the clinic and community setting. Programming will include both in-person and virtual formats. The Clinic Health Mentor will work closely with the Hopkins High School Health Mentor to help deliver sexual health curriculums, with travel to Hopkins High School. The Health Mentor will largely work independently and must collaborate to a high degree with myHealth for Teens & Young Adult clinic staff, Hopkins School District staff, and other community partners.

EDUCATION TEAM

We engage community education holistically, utilizing both trauma-informed and positive youth development approaches when working with young people and community partners in order to support and establish safe learning environments, healthy decision-making, engaged youth and supportive families.

Hours: 40 hours/week

Evening hours on Tuesdays and 4 hours every other Saturday

This is a benefits eligible position.

This is a contracted position until 12/31/2025, with the possibility of renewal for the following year.

ESSENTIAL JOB FUNCTIONS

  • Provide one-to-one education using evidence-based intervention motivational interviewing. Provide support, and developmentally appropriate information to young people who are seeking guidance, resources, and mentoring on topics such as relationships, sexual health, accessing health care and various other related sexual health issues
  • Identify, develop, adapt and revise, as needed, programming, materials and activities that support student’s sexual health and wellbeing
  • Facilitate evidence-based health education presentations and youth development groups for students, young adults, and parents in Hopkins High School, partnering schools and other youth-serving organizations; includes collaboration with other health educators; educational presentations are off-site, and travel is required
  • Assist clinic staff in client reproductive health education and follow up
  • Collaborate with clinic staff and administration to ensure all health mentor services are confidential, culturally relevant, easily accessible, and supported across the school
  • Collect and report data regularly including monthly reports, complete accurate compilations of evaluation and statistical data for reporting
  • Maintains strictest confidentiality; adheres to all HIPAA guidelines/regulations
  • Build productive relationships and collaborate with myHealth staff, Hopkins School District staff, and community organizations on programming and services, working groups, task forces, and other groups
  • Provide leadership in advancing sexuality education programs, safe and supportive schools, and policies that support young people’s sexual and reproductive health in the school and community
  • Engage in continuous professional development to ensure services are culturally relevant, trauma-informed, inclusive, accessible, and using the most current information available
  • Manage programming independently, with limited supervision

QUALIFICATIONS

Required:

  • Bachelor’s degree in youth development, social work, sociology, psychology, women and gender studies, public health, education, nursing OR extensive (2+ years) volunteer and/or teaching experience with pre-teens, teens and/or young adults.
  • Experience in public health, education, youth work or program facilitation, working with young people grades 6 – 12
  • In-depth knowledge of sexual health and adolescent development. Comfort discussing sexual and reproductive health issues
  • Ability to work flexible hours including Monday and Tuesday evenings and occasional weekends
  • Ability to work collaboratively with other educators, clinic staff, volunteers and community members
  • Strong analytical and problem-solving skills
  • Ability to work independently as well as part of a high performing team
  • Committed to the mission, vision and core values of myHealth

Preferred:

  • Previous experience with Motivational Interviewing
  • Previous experience working in a clinic setting
  • Experience and confidence in speaking in front of large groups of people

REQUIRED LICENSES / CERTIFICATIONS

  • Must have valid MN driver’s license, car insurance and own vehicle
  • Must complete Mantoux testing

PHYSICAL REQUIREMENTS / WORKING CONDITIONS

Environmental/Working Conditions: Work is performed primarily on site at myHealth for Teens & Young Adults and Hopkins High School.

Physical Demands: May require occasional lifting of supplies and equipment of up to 50 lbs.
