WHAT WE DO

myHealth for Teens & Young Adults clinic-based programs offer youth and young adults, ages 12-26, comprehensive, preventative and youth-focused medical services, mental health services, community outreach and education, and nurse home visiting for pregnant and parenting teens. Our clinic is located downtown Hopkins and our services are offered without judgment in a caring and safe environment.

JOB SUMMARY

The Clinic Health Mentor position is responsible for providing a range of sexuality education programming and serving as a resource and support around sexuality and health topics within the clinic and community setting. Programming may include both in person and virtual formats, local travel required. The Health Mentor will largely work independently and must collaborate to a high degree with myHealth for Teens & Young Adult clinic staff, Hopkins School District staff, and other community partners.

EDUCATION TEAM

We engage community education holistically, utilizing both trauma-informed and positive youth development approaches when working with young people and community partners in order to support and establish safe learning environments, healthy decision-making, engaged youth and supportive families.

Hours: 40 hours/week

M/T: 12-8pm shift, W-F: 9-5pm

This is a benefits eligible position.

ESSENTIAL JOB FUNCTIONS

  • Provide one-to-one education using evidence-based intervention motivational interviewing. Provide support, and developmentally appropriate information to young people who are seeking guidance, resources, and mentoring on topics such as relationships, sexual health, accessing health care and various other related sexual health issues
  • Identify, develop, adapt and revise, as needed, programming, materials and activities that support student’s sexual health and wellbeing
  • Facilitate evidence-based health education presentations and youth development groups for students, young adults, and parents, in schools and other youth serving organizations; educational presentations are off-site, and travel is required
  • Assist in recruiting and facilitating a youth leadership council, peer education group, or equivalent ongoing small group program
  • Collaborate with clinic staff and administration to ensure all health mentor services are confidential, culturally relevant, easily accessible, and supported across the school
  • Collect and report data regularly including monthly reports, complete accurate compilations of evaluation and statistical data for reporting
  • Maintains strictest confidentiality; adheres to all HIPAA guidelines/regulations
  • Build productive relationships and collaborate with myHealth staff, Hopkins School District staff, and community organizations on programming and services, working groups, task forces, and other groups
  • Provide leadership in advancing sexuality education programs, safe and supportive schools, and policies that support young people’s sexual and reproductive health in the school and community
  • Engage in continuous professional development to ensure services are culturally relevant, trauma-informed, inclusive, accessible, and using the most current information available
  • Manage programming independently, with limited supervision

QUALIFICATIONS

Required:

  • Bachelor’s degree in youth development, social work, sociology, psychology, women and gender studies, public health, education, nursing OR extensive (2+ years) volunteer and/or teaching experience with pre-teens, teens and/or young adults.
  • Experience in public health, education, youth work or program facilitation, working with young people grades 6 – 12
  • In-depth knowledge of sexual health and adolescent development. Comfort discussing sexual and reproductive health issues
  • Ability to work flexible hours including Monday and Tuesday evenings and occasional weekends
  • Ability to work collaboratively with other educators, clinic staff, volunteers and community members
  • Strong analytical and problem-solving skills
  • Ability to work independently as well as part of a high performing team

Preferred:

  • Previous experience with Motivational Interviewing
  • Previous experience working in a clinic setting
  • Experience and confidence in speaking in front of large groups of people

REQUIRED LICENSES / CERTIFICATIONS

  • Must have valid MN driver’s license, car insurance and own vehicle
  • Must complete Mantoux testing

Licensed Mental Health Therapist

Reports to: Mental Health Program Manager

JOB SUMMARY

Individual will be responsible for providing diagnostic assessment, treatment planning and therapy services to adolescents, young adults ages 12-26 and their families at the Hopkins clinic, satellite, and school-based settings. Responsibilities include therapy with clients and family, parent education, possible group therapy, engaging in outreach and community education activities and related paperwork.  This position may include time spent on outreach and networking activities to build community knowledge and client base. This position requires the following hours:

  • Part-time or Full-time, hours negotiable, required one evening a week and every other Saturday 10-2. Benefits eligible with a minimum of 20 hours a week.

ESSENTIAL JOB FUNCTIONS

  • Primary job is to provide direct therapy services to adolescents, young adults and their families at the Hopkins clinic, school-based settings, and community partner locations.
  • Occasional outreach, consultation and other professional presentations to school systems, referral sources and other related community groups
  • Must have reliable transportation and a valid MN State Driver’s License to travel and provide off site mental health services at Satellite clinics, or local schools.
  • Carry out all necessary administrative activities regarding charting, billing, program reporting and scheduling
  • Attend scheduled Mental Health program, all-clinic and all-staff meetings on the second Thursday every other month

OTHER FUNCTIONS AND RESPONSIBILITIES

  • Deliver mental health presentations to youth and adults in small and large groups
  • Meet with appropriate personnel and assess local school needs for weekly student support groups and implement those groups
  • Consult and collaborate with other myHealth clinic staff to assess the needs of clients in the medical clinic.
  • Maintain strictest confidentiality; adhere to all HIPAA guidelines/regulations
  • Conduct self in accordance with myHealth’s employee manual, internal purpose and core values
  • Build referral network through community meetings and outreach activities
  • At all times, support the mission, objective and core values of myHealth
  • Report workplace hazards to your supervisor and safety officer
  • Other duties as assigned by Clinic Director or Executive Director

QUALIFICATIONS

Required:

  • Master’s degree in Social Work, Marriage and Family Therapy or Psychology from an accredited University
  • Previous experience working as a licensed clinical social worker (LICSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Psychologist (LP) in the state of MN
  • Previous experience working with adolescents and young adults
  • Demonstrated knowledge of age related growth and development principles
  • Demonstrated ability to develop and maintain therapeutic relationships with adolescents and young adults
  • Ability to build and sustain effective collaborative relationships with colleagues, staff and community partners
  • Ability to work one evening and every other Saturday 10-2
  • Ability to work effectively with individuals from a variety of economic, social and culturally diverse backgrounds
  • Technical proficiency in Excel, Word and Outlook
  • Excellent verbal and written communication skills
